You are looking for a psychologist-psychotherapist and/or your GP has referred you to me.  You are eager to make your first appointment, but you are not yet sure whether you have come to the right place/are with the right person. Well, that's what this pre-intake form is for. By completing this form, you will not only place yourself on the waiting list to be contacted as soon as a place becomes available, but I can also let you know, based on your answers, whether I am the right person to help you further.
